Q. Can I fax UAGC-CS my order or do I need to submit it online?
A.

Yes, completed requisition can be faxed to 520-626-7701. Attention: Gina Delgado, Clinical Services Coordinator.  For additional questions regarding test orders, please call 520-626-5502.

Q. What personal information will I need to provide?
A.

Your physician will complete a test order requisition that will require information such as your complete name, address, date of birth and insurance information.  A copy of previous lab tests and medical records may also be requested.  Rest assured that all information is secure and private.  UAGC-CS strictly follows HIPAA guidelines.

Q. Do I need to sign a consent form?
A.

Yes, a consent form must be completed and signed in addition to the completed requisition form prior to performing any tests. This should be provided by the ordering physician.

Q. Do I need a doctor to order my test?
A.

Yes, UAGC-CS cannot process any test requests without a signed physician order on file.

Q. Will my insurance cover the cost of testing?
A.

Some insurance companies cover genetic testing. Benefits and eligibility vary with different plans.  It is recommended that you contact your insurance company to obtain benefit, co-pay, co-insurance and deductible information for your plan. UAGC-CS will assist with the insurance authorization process prior to performing any test by submitting a pre-authorization request to your insurance.  If the test is not covered, there are payment options available.

Q. Is UAGC-CS an in-network provider for my insurance?
A.

UAGC-CS is Medicare certified.  We currently are not contracted with commercial carriers.
